if outside part of the sentence is in past tense then inside sentence change in past here is not going this thing why

Solution:

If the reported speech in direct speech is in present tense, it changes to past tense in indirect narration. When changing from direct to indirect speech or vice versa, the words remain the same. So, 'twice a day' is the correct usage.
