Wrong option (Question:147690.4)

Choosed different option which is correct also but showing different option that was wrong...Fix it as soon as possible.

Question:
DIRECTIONS: A sentence/a part of the sentence is underlined. Four alternatives are given to the underlined part which will improve the sentence. None of us came to know that how it was happened.

Options:
A) was happen
B) has happened
C) had happened
D) No improvement
Solution:
Ans (c)

When there are two actions in the past ,the previous one takes past perfect form and the next one is in simple past.

"came" simple past

"had happened" past perfect
