Option C would be correct only if you replace "Do" with "Is"

Question:
DIRECTIONS: In the following question, a sentence has been given in Active/Passive Voice. Out of the four alternatives suggested, select the one which best expresses the same sentence in Passive/Active Voice.

Options:
A) What I mean is that understood by him
B) Was what I mean understood by him
C) Is what I mean understood by you?
D) What I mean is understood by him
Solution:
Ans: (c) Is what I mean understood by you?

would be the correct passive form of the given sentence.

It seems to be right in this case as if we use mark option D then the meaning of the sentence will change.
