As far as I know 'One of the problems that occur' is correct.

Here 'that' works on 'problems' not on 'one'.

The above is just like the usage of 'who/which'.
'A sits to the left of B who sits to the right of C'. Here, 'who' works only on B.

One of the problems that occurs as a result of coal mining is the increasing incidence of lung cancer.

Options:
A) One of the problems that 
B) occurs as a result of coal mining
C) is the increasing incidence of lung cancer.

Solution:
Ans: (b) The pronoun has the same number as its antecedent, which is ‘problems’. Since ‘problems’ is plural, ‘that’ is also plural, and so the verb associated with it must agree with ‘problems’. Problems occur. So, the correct usage is “one of the problems that occur”
